The employer must disclose to the consumer the fact that a report may be procured; what is the purpose of the investigation; the fact that the report may delve deeper into the significant information of the consumer's character, reputation, personal characteristics, and the consumer's name and address, and telephone number of the investigative consumer reporting agency.
